You have the right to receive a Good Faith Estimate explaining how much your care will cost

Under federal law, health care providers must give patients who don't have insurance, or who are not using insurance, an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.

How this applies at Clarity

Clarity is cash-pay only, so every patient is a self-pay patient and every patient has this right. Your estimate is issued by Clarity Life Medical P.A.

Your estimate covers your membership fee, including the Clarity Gut Panel, which is included at no separate charge. Your membership is month-to-month. Because an estimate for recurring services may cover up to 12 months, yours shows what you would pay across 12 months; if you cancel sooner, you pay only for the months your membership was active.

Supplements and any additional laboratory testing are purchased directly from a third-party vendor. Clarity does not bill or collect for them, so they are not part of your estimate from Clarity. That vendor shows you those prices before you purchase.

How to get yours

We send your estimate in writing after you enroll. You can also request one at any time, including before you enroll or schedule anything, by emailing hello@getclarity.health or calling 720-902-3050. We will send it within 3 business days, by email or on paper if you prefer. If a new service is ordered that carries a charge not already in your estimate, we will send an updated one.

If you are billed more than your estimate, contact us first at hello@getclarity.health or 720-902-3050. You may also start a dispute resolution process with the U.S. Department of Health and Human Services within 120 calendar days of the date on the original bill.
